Family: Married to Lindy - as of 4th December 2018, for 46 years.
Two sons, Charles and Edward, married to Katherine and Clare respectively. Each couple has a daughter: Saige (aet 10) and Lydia (aet 8) respectively, and Ed and Clare have a son, Bradley (aet 3), as of December 2017, and have a second son Andre, as of 12th March 2018.
Sport: Surfing on Shortboards on Fridays to Sundays, throughout the year, except during holidays, when generally this becomes daily. Preferred board is a 6ft 10in tri-fin, designed by John Carper, and known as 'The Equalizer'. John also designs for Kelly Slater. I recently acquired my first high volume shortboard, of 5 foot 11 inches, which I am still trying to learn to ride. I may have to go back to my Mini-mal even a Mal.
Music: Kit Drummer for 30+ years: I generally do two gigs a fortnight, usually nowadays on Christian music. In June 2018, I become competent (well, relatively) ambidextrously on the kit. I now practise silently on an electronic drum kit.

TEACHING EXPERIENCE:
Undergraduate
In the Ophthalmology Clinics at Concord Hospital (University of Sydney) and Prince of Wales Hospital (University of New South Wales), I have taught all years of Medicine but mainly Medicine V and VI.
Over more than 42 years, many young University-based Medical students, and Postgraduate doctors, have attended my Operating sessions to learn something of the technology and management of cataract patients. Until 2002, I regularly had the same groups attend also my Ocular Plastics surgical sessions.
I have been running the Sydney Ophthalmology Writers Group (WG or SOWG) in Sydney for over 30 years. WG is currently a 2-3 hour second-weekly event held at my home in Sydney, usually from 10am on a Saturday morning. The Writers Group is attended by Medical students, Scientists, Ophthalmology Pre-registrars and Registrars, as well as Consultants and Academics in Ophthalmology, Internal Medicine, Neurosurgery, Vascular Surgery, Haematology, Radiology, Gastroenterology and Pathology, with many more Specialists, invited to help the teaching, planned for the future.
The primary aim of the WG for Postgraduate doctors is to help potential Ophthalmology Registrars obtain publications prior to their RANZCO interviews, possibly to assist their credibility in obtaining a job in RANZCO training. The secondary aim is to enjoy the breadth of Medicine and Ophthalmology, partly by achieving Medical Inter-Specialty interactions and subsequent publications. The tertiary aim is for more junior and more senior Ophthalmologists and Trainees to meet and learn from each other in a fun environment.

Postgraduate and Registrar Training
I held Postgraduate and Registrar teaching sessions at Concord Hospital and then at Sydney Eye Hospital in Woolloomooloo in the 1970s, and many of the current Senior Consultants in Australia were my students at one time. Following (now Professor) Melbourne’s Justin O’Day’s lead, I instigated the Neuro-ophthalmology Ward Round in Sydney. (Please see paragraph 8, below, in this section).
I worked and operated at Addenbrookes Hospital in Cambridge, England in 1978, gaining surgical and medical experience there from numerous world-recognised Ophthalmological Consultants, including Mr John Cairns, who invented the Trabeculectomy, Mr John Scott, who pioneered aspects of Vitreoretinal surgery, especially the use of Silicone oil, and Mr Peter Watson, who was a leader in some Ophthalmological immune disorders, especially of the sclera. I also contributed to the development of a Neuro-Ophthalmology meeting in Cambridge, under the auspices of Mr John Keast-Butler FRCS.
In the Oxford Eye Hospital, England, and also at the Churchill Hospital in Oxford, I set up the first Neuro-Ophthalmology meeting, and organised teaching and case presentations while I was there in 1979. I was impressed to work with the legendary Professor Tony Bron, a renowned Corneal Specialist.
I also set up a regular teaching session in relation to Medicine, Surgery and Ophthalmology while working in The Gambia, West Africa, during my stay there as Consultant Ophthalmic Surgeon for that country in 1979.
I have taught Neurosurgeons and Neurologists in formal clinical sessions and on Neuro-Ophthalmology ward rounds over the last 38 years.
In the last seven years (till 2019), I have undertaken teaching Basic Physician Trainees (BPTs) (FRACP candidates in Internal Medicine), and Medical Students and JMOs in an attempt to allow them some to achieve competence in some of the more important features of Neuro-Ophthalmology, including Neurological Diagnosis as it relates to the visual system. This has been reported by some of them to be helpful in their final Viva assessment for their FRACP examination.
I have undertaken numerous teaching sessions and seminars for Ophthalmology Registrar candidates, both formally and informally, and at the Annual Registrars Conference in January in Sydney in both Ocular Plastics and Neuro-Ophthalmology. The last occasion was in Sydney in January, 2016.
Every week for the last 39 years, on a Monday evening at about 5.30pm, excluding various holidays and absences, I have conducted Neuro-Ophthalmology Ward Rounds (NOWRs) around Concord Hospital (till 1996), and then at Prince of Wales Hospital (from 1996 until the present, indeed 2019). In a teaching role for them at the NOWRs, some of these sessions have been attended by eminent visiting Consultant Australian and International Neurologists, Neuro-ophthalmologists and Neurosurgeons, who have taught the candidates in the usual Viva fashion.
These sessions have been supported by live cases provided by and sometimes referred by Neurologists, Neurosurgeons, Radiation Oncologists, Medical Oncologists, Rehabilitation Specialists, Pulmonologists, Cardiologists, Emergency Medicine Specialists, Plastic Surgeons, Nephrologists, ENT Surgeons, Dermatologists, Infectious Diseases Specialists, Immunologists, Haematologists and many others.
The documented patient consultations from an 18 month session of the NOWR was presented at our international Neuro-Ophthalmology Society of Australia (NOSA) meeting in 2012. Following another 18 months of cases so seen, a further presentation was given at the RANZCO Annual Scientific Meeting in 2013 in Hobart, and at the September 2014 NOSA Meeting in Canberra.

Continuing Education in Ophthalmology
In the course of the last 38 years, over 278 publications have been produced by myself, almost invariably in collaboration with colleagues. All of these have been published in peer-reviewed literature, and precisely 236 of them to date have been documented in PubMed (MedLine). Included in these 278 publications have been several book chapters.
As a consultant, I attended weekly Neurology Grand Rounds at Concord Hospital for 16 years. I have attended, and presented topics of Neuro-ophthalmological import, as ‘Visiting Consultant in Neuro-ophthalmology’, at Neurology Grand Rounds over the last 23 years at Prince of Wales Hospital. Here, I learn from, and attempt to educate, Neurologists and Physicians, as well as Neurosurgeons, and Ophthalmologists, in Neuro-Ophthalmology.

Community Engagement
I have spoken to numerous community groups regarding basic concepts in Ophthalmology, Ophthalmological Diagnosis, Ophthalmic Surgery, and Neuro-ophthalmology. These include Men’s Groups both in Chatswood (Sydney) and Forster (country NSW), as well as the Council for the Aging, and in 2014 a Community group at the War Memorial Hospital at Waverley, Sydney.

Optometry
I was awarded my PhD in 2008 at the University of New South Wales in the Department of Ophthalmology. In this endeavour, some of the laboratory work was done at the UNSW Department of Optometry in terms of Tear osmometry and Lid tensometry.
A total of 35 associated papers was produced in both these environments over the course of the PhD, and related to it.
I have lectured to Optometrists or equivalent over the last 43 years, teaching them mainly in the realm of Anterior segment disorders including Cataracts, as well as Neuro-Ophthalmology and Ocular Plastics surgery.
I have been closely involved with the Ophthalmic Surgery Centre, Chatswood, now known as Chatswood Private Hospital, in presentations to General Practitioners and Optometrists, at numerous GP and Optometry teaching days over the last 25 years.
I am currently consulting, pro bono, on the basis of a TeleMedicine diagnostic programme with a Mobile Optometry Unit, run by an Optometrist in Sydney.
Since July 2019, I have commenced pro bono work for MedeVac, a group supporting asylum seekers and prisoners in TPNG, and particularly in Nauru. This involves assessing such patients via a remote translator, and forming an opinion re their Ophthalmological and Neuro-ophthalmological status over the phone. This is then conveyed to MedeVac officials and lawyers who, armed with this knowledge, attempt to help these patients

PEER REVIEW AND AUDIT
I have been a reviewer of Ophthalmological literature for over 39 years, with journals including the Journal of Cataract and Refractive Surgery, Clinical and Experimental Ophthalmology, Ophthalmology, Ophthalmic Plastic and Reconstructive Surgery, British Journal of Ophthalmology, Medicine Today, and numerous others.
My personal audit has mainly consisted of the results of generally large series of surgical procedures both from the Ocular Plastics Surgical point of view and the Cataract Surgical point of view. To this have been added my Teaching activities at all academic levels and Publications in journals and textbooks.
I continue to attempt to create an evaluation of my own current new ideas, and Surgical and Visual Outcomes, in Cataract surgery nowadays. Recent publications, including up to 2018, have shown this.
As well, I attempt to evaluate what we are already doing surgically, in order to see if the standard Visual outcomes can be improved upon. Indeed, I continually show that over 92.3% of patients who have no pre-existing corneal, lens capsular, vitreous, retinal, optic nerve or intracranial pathology should achieve Corrected Distance Visual Acuity (CDVA) of 20/12 (6/4) at one month following cataract surgery. For many interesting reasons, this is not always very popular amongst cataract surgical colleagues around the world. However, since the second half of 2018, the notion of excellent postoperative CDVAs, at least among the younger Ophthalmologists, is becoming more acceptable, and indeed a worthy aim. The lack of acceptance of my having achieved these visual outcomes in large cohorts is currently being addressed in my DSc (Doctor of Science, at UNSW). I am working towards the DSc being submitted by late 2019.
As of most recently, I do not usually attempt to evaluate Surgical outcomes alone, as around the world these are generally excellent, with the very notable exception of Postoperative Cataract Surgical Endophthalmitis (POE). It is clear, and numerous of my publications support this, that POE is generally underreported. One reason for this is that it usually results in an inadequate Visual outcome, at least by comparison with the patient’s preoperative CDVA. I have currently over 26 publications on MedLine which attempt to address POE, and its prevention.
My current and ongoing Audit relates to Intravitreal Injections of VEG-F Inhibitors, given mainly in Sydney at the Chatswood Rooms and Prince of Wales, as well as at Katoomba, Fairfield and Campbelltown. I am auditing my Intravitreal Injections for Wet Macular Degeneration, Retinal vascular occlusions, and Diabetic Retinopathy. The current priority is the PIVIC Study, where a ‘Prospective IntraVitreal Injections Circumferential Study’ is being carried out injecting at twelve positions around the eye, avoiding 12, 3, 6 and 9. In this study, injections of various VEG-F inhibitors are given as above, aiming to note the efficacy and safety of this procedure. I have designed a new recording technique which enables Intravitreal injections to be given seamlessly without repeatedly traumatising the scleral pars plana in just one or two locations.
I am also auditing my expanding series of Temporal Artery biopsies (TABxs), performed in order to facilitate the diagnosis of Temporal Arteritis (aka GCA). I have worked, collaborated and presented with Dr Anna Finniss, Rheumatologist of Chatswood, and her Rheumatological colleagues, including Dr Ann Cheung, and with Dr Alexandra Allende, who is a dedicated Histopathologist of Douglass Hanly Moir. As a small item of interest, I performed three TABxs in the week commencing 141018, and two of three were positive.



CLINICAL EXPERIENCE
I was involved in general Medicine and Surgery at Royal North Shore Hospital as a Junior Resident and Senior Resident. {These are now currently known as, respectively, ‘Intern’ and ‘RMO 1’ (= Resident Medical Officer #1), as well as PGY1 and PGY2}.
I did most aspects of Ophthalmic surgery as a Registrar at Concord Hospital from 1974, and then at Sydney Eye Hospital until graduation, where I completed my formal Examination-based training in April 1977, after arriving there in January 1977.
In Addenbrookes Hospital, Cambridge, I was involved with German translation for Mr John D. Scott, who was a prominent early Vitreoretinal surgeon. He used liquid silicone for re-attaching detached retinas in those days. In this situation, and in 1978, I was able to create some slides with German subtitles for presentations given by John in a German-speaking environment in Europe.
I performed a major Retinal Pigment Epithelium literature research topic Review and presentation while in Cambridge, receiving information from around the world including from Professor Richard W. Young of Baltimore.
In Cambridge, I undertook standard Cataract surgery from those days, as well as some Ocular Plastics, Trauma surgery, Orbital surgery, and Lacrimal surgery.
The same applied in Oxford, where, under the influence of Professor Tony Bron, I was involved in corneal and ocular surface disorders.
I set up Neuro-ophthalmology Meetings in both Cambridge and Oxford while I was there. This involved Neurologists, Neurosurgeons and Ophthalmologists.
I was the Ophthalmic Surgeon for The Gambia, West Africa, during March-June 1979, carrying out many Cataract operations, Trabut procedures for trachoma, and I suspect the first corneal graft, the first DCR, and the first fascia lata suspension procedure for ptosis, as well as orbital surgery, trauma surgery, the first Intraocular Foreign Body removal, and numerous other procedures.
Appointed as a VMO at Concord Hospital, from 1980, my time was divided between managing Neuro-Ophthalmological patients, and doing anterior segment surgery, mainly cataracts and pterygia and some corneal surgery, but with two thirds of the surgical workload being Ocular Plastics. At Concord, I set up and developed the first Ocular Plastics Unit, the second one in Sydney at that time.
When I arrived at Prince of Wales in 1996, my time was totally devoted to Ocular Plastics surgery, as first Director of the Ocular Plastics Unit, and to doing and teaching Neuro-Ophthalmology. Over the years until now, I have performed regular after-hours procedures in emergency Orbital, Lacrimal and Eyelid surgery, as well intraocular surgery. I ceased formal intraocular surgery on the 10th August, 2016, at 2pm, after over 43 years of operating, but continue doing my one OR-based operation of Temporal Artery biopsy.
I was involved with seeing and operating on children at Sydney Children’s Hospital from the Ocular Plastics point of view from 1996 until 2002.
From then, when there was no other Ocular Plastics cover in the hospital, and which was done almost invariably by Dr Geoff Wilcsek, who succeeded me as Director of the Ocular Plastics Unit, I did some of the emergency after hours Eyelid and Orbital surgery, as well as General Ophthalmology.
I was the first Ophthalmic surgeon to set up and commence Ophthalmic surgery at Dalcross Private Hospital in 1980, due to the withdrawal of Medical and Surgical services related to the institution of Australian Medicare. I was followed by numerous other Ophthalmic surgeons over the next few years, and worked there doing regular Ocular Plastics and Cataract surgery until 2000.
I then moved my private surgical practice entirely to Ophthalmic Surgery Centre, Chatswood where I had an all-day Wednesday operating list which consisted largely of Cataracts, Temporal artery biopsies and Pterygium surgery. As mentioned above, while I ceased almost all major Ophthalmic surgery on 10th August 2016, I still do some simple lid procedures such as chalazia, in the Rooms, as well as anti-VEGF injections. As mentioned above, my only other surgical procedure is doing Temporal Artery Biopsies, for which I am available continually, and which are generally done at Chatswood Private Hospital.
Indeed, Ophthalmic Surgery Centre is now known, following updating in place and services since 2016, as Chatswood Private Hospital. About 50 surgeons work there, most being Ophthalmic Surgeons, along with ENT surgeons and a few others, including myself with just Temporal artery biopsies.
I transiently operated at Castlecrag Private Hospital, doing Orbital, Lacrimal and Eyelid surgery, as well as Cataract surgery. When their Administration decided in the late 1980s that phacoemulsification was not the way forward regarding cataract surgery, I left and moved all my work back to Dalcross Hospital. As stated above, I then moved my surgery to the Ophthalmic Surgery Centre, Chatswood, now named Chatswood Private Hospital.
I have a wide experience in Anterior segment and Ocular Plastics surgery, which has proved very satisfying and has appeared to have benefitted patients.
I have visited Burma as an Ophthalmic Surgeon twice between 2010 and 2014 for a stay on each occasion of between 10 and 14 days. These trips are carried out under the auspices of the Myanmar Eye Care Programme, run by Dr Geoff Cohn OAM, Ophthalmic Surgeon of Sydney. We consulted and operated in fairly remote hospitals in Myanmar, mostly doing cataract surgery, but at times performing Ocular Plastic surgery.
I travelled to Hanoi, Vietnam for a week in November 2014 to teach, and examine in, some Neuro-Ophthalmology to the local Ophthalmologists, Neuro-Ophthalmology Trainees, and Physicians. This appeared to be well received, and opportunities have arisen for further teaching and interaction with the Vietnamese Medical fraternity since my arrival home.
As mentioned above, I ceased operative Cataract Surgery on 10th August, 2016. I continue to consult at Chatswood Eye Specialists, and to do Temporal Artery biopsies at Chatswood Private Hospital. I have also set up a consultative group which manages Temporal Arteritis. This consists of at least four Rheumatologists, a dedicated Ophthalmic Histopathologist, all mentioned above, with myself as Ophthalmic Surgeon and diagnostician. The aim of this group is to achieve early diagnosis and management of Temporal Arteritis, planning to obtain a Temporal Artery biopsy result and a Rheumatological consultation within 24 hours of the referral.
Having ceased Cataract surgery, I have been attending second-weekly early Monday morning Registrar tutorials by one of the VR surgeons at Prince of Wales Hospital. There, Dr John A. Downie, Vitreoretinal Surgeon, teaches on Medical and surgical Retina. This commenced for me from 2016. I attended eight major talks on Medical Retina at the November 2016 RANZCO ASM in Melbourne. I was privileged to join, as a student, the new and older Registrars from around Australia in the three day Ophthalmology Registrars Annual Conference at Sydney Eye Hospital in January 2017, learning the A-Z of Medical Retina. I have since commenced anti-VEG-F injections for patients in Sydney in the Rooms at Chatswood,Katoomba, Fairfield and Campbelltown, and in the public clinic at Prince of Wales.
I attended more Medical Retina conferences during 2017-2019, notably a dedicated Medical Retinal conference in Adelaide, plus a further Medical Retinal conference in Queensland in August, 2017. I attended and contributed to an Eylea (Bayer) conference in Sydney in late August 2017. I also attended another Eylea Conference in October, 2017, in Sydney. These Medical Retina conferences have continued till 2019, and are ongoing.
I attended further teaching on Medical Retinal disease at the American Academy of Ophthalmology meeting in New Orleans in November 2017. I attended, and presented at, the RANZCO Annual Scientific Meeting in early November 2018 in Adelaide.
I consulted and taught in General Ophthalmology, Neuro-ophthalmology and Medical Retina and Ocular Plastics in Katoomba, NSW, on a weekly basis, until November 2018.
From the 5th December 2018, I will be transferring the Katoomba work to Campbelltown, NSW, where, every second Wednesday, I work in the King St practice of Dr Julie Yip. I am the only Ophthalmologist who is doing anti-VEGF injections in this practice. I spend my Tuesdays consulting at City West Eye Centre in Fairfield, with Dr Sarya Saks. I will be working on alternate Wednesdays in Dr Robyn Armstrong’s practice in Blacktown from September 2019.

2. In addition to the more than 278 articles already published and in print, I have currently over 30 studies and papers in preparation for submission for publication in peer-reviewed journals around the world.
While some of this latter group are just going through Ethics approval, some are nearly bearing fruit.
These articles will almost certainly, at least hopefully, appear in the published literature with the passage of time.
One particular paper on ‘Refraction’ had its data collected at the Perth RANZCO in November 2017. Of the 1000 attendees, there were over 230 respondents to a questionnaire. This should be a very instructive publication. It is currently under consideration by an international Ophthalmology journal.
