I'm a clinical pharmacist working in ambulatory care setting in Singapore General Hospital. Clinical services I provide include medication therapy management, anticoagulation monitoring and titration and medication reconciliation for patients on hemodialysis. I also have interest in vitamin D testing and vitamin D dose optimisation to achieve target levels. My research interests include medication management and optimisation in patients with complex conditions and polypharmacy.
