Author and presenter of more than 60 scientific papers in internal medicine, geriatrics, and

implementing the Quadruple Aim in healthcare to promote higher quality of care, improved

growing geriatric population.

Medical Director of the Geriatric Ambulatory Clinic, Internal Medicine Section Chief, Associate

and Internal Medicine. He actively contributes to the implementation of the ACGME-required

At Eisenhower, Dr. Moshiri founded and developed the Geriatric Division, establishing key
initiatives such as the Geriatric Outpatient Primary Care Clinic, Dementia and Memory Care
Clinic, Geriatric-Focused Wound Care Clinic, Skilled Nursing Facility section, Geriatric
LGBTQ+ Clinic, and expanded home care services. These programs are built on the 5Ms
framework to promote age-friendly, patient-centered, cost-effective, and coordinated care,
aligned with the Quadruple Aim objectives.
As leader of Eisenhower Health’s Age-Friendly initiative, Dr. Moshiri guided the Geriatric
Ambulatory Clinic to achieve recognition on April 30, 2025, by the Institute for Healthcare
Improvement and its partners (The John A. Hartford Foundation, American Hospital
Association, and Catholic Health Association of the United States) as an Age-Friendly Health
System – Committed to Care Excellence, advancing the 4Ms (What Matters, Medication,
Mentation, and Mobility). This recognition reflects his success in transforming ambulatory
geriatric care and provides the foundation for expanding these initiatives into inpatient care to
meet CMS Age-Friendly standards.
Dr. Moshiri comes from a family of physicians, so his career path was clear from an early age.
“I was surrounded by doctors in my family,” he recalls. “From an early age, I was challenged to
make a difference with my life’s work and to improve people’s lives.” During residency, he was
drawn to geriatric medicine for its unique challenges and complexities. “I believe mutual respect
is foundational in working with my patients,” states Dr. Moshiri. “I approach my patients with a
5M Framework, focusing on 1) Mobility, 2) Mentation, 3) Medication, 4) What Matters most,
and 5) Multiple complex and chronic conditions. My aim as a physician leader is to provide a
holistic, patient-centered, age-friendly, and cost-effective approach to improve quality of life
while preserving the dignity of older adults. Ultimately, my vision is to position Eisenhower
Health as a national leader in age friendly, value-based care and to serve as a catalyst for
systemic change in geriatric medicine. I aim to build a care delivery model that upholds dignity,
independence, and quality of life for older adults, transforming how we care for aging
populations locally and nationally.
