Head, Shoulders, Knees, and Don’t Forget the Toes: A Case of CLL Developing to BCL


Abstract

Background & Introduction
Chronic lymphocytic leukemia, B cell lymphoma, lower extremity, Richter transformation

 

Objective
Examine how the progression of CLL to BCL in this patient can inform providers of how to effectively evaluate patient’s with CLL while emphasizing the need to perform a detailed physical exam.

 

Methods
Case report on a 76 yo Male who presented to a Florida ED with an eye abscess. Case data was collected from the patient’s medical record following informed consent that the patient’s information would be used for a case report.

 

Conclusion
This case demonstrates a rare occurrence of CLL progressing to BCL in the distal extremities. Additionally, this case emphasizes the need for thorough examination of the distal extremities and digits to ensure pathologies unassociated with a patient’s chief concern are not missed. This case report contributes to the existing knowledge of CLL presentation and how it can develop into a more malignant BCL in various anatomical regions.
