Abstract
This report presents a rare case of highly resistant S. epidermidis in a 62-year-old patient who developed chronic osteomyelitis, abscess, and cellulitis, even in the absence of prosthesis or hardware, and despite multiple surgical debridements. It explores some of the risk factors that may have predisposed him to developing chronic osteomyelitis and provides some insight into managing cases with more antibiotic-resistant organisms. Ultimately, this patient’s past medical history of peripheral arterial disease, neuropathy, and HIV—complicated by poor wound care, chronic antibiotic use, and ongoing weight-bearing status—may help explain why he may have developed a case of chronic osteomyelitis and cellulitis. The use of daptomycin should be considered in cases involving highly antibiotic-resistant organisms or which are more chronic.
