Abstract
When to start or restart anticoagulation after head trauma? In this case, the patient was on anticoagulation for cardiomyopathy, so the question arises, was there a need to start anticoagulation in the hospital setting? The decision to start anticoagulation at that time was based on the patient’s neurological status, duration since the head injury, initial CT head imaging, and overall clinical presentation. The patient was at his baseline neurological status with no acute changes reported from nursing staff or family. Factor Xa inhibitor was not resumed in the hospital setting due to its long half-life (5-9 hours). Heparin was started as it would be easier to reverse in case anticoagulation needed to be stopped rapidly.
