Understanding Esophageal Intramural Pseudodiverticulosis: A Systematic Review


Abstract

 

INTRODUCTION:

Esophageal intramural pseudodiverticulosis (EIP) is an extremely rare condition in which the pseudodiverticula expands from the esophageal lumen to the outer wall of the esophagus which is diagnosed endoscopically, radiologically, or pathologically and presents with intermittent or progressive dysphagia with an unknown pathophysiology. It’s a unique disease with peculiar findings and treatment.

 

AIM:

To determine the epidemiology, risk factors, and complications of Esophageal intramural pseudo diverticulosis (EIP) a rare condition of GI system.

 

METHODS:

A meticulous search was performed using different MeSH keywords in PubMed, European PMC, Cochrane and Science Direct. A total of 56 articles were found out of which 14 articles (13 case report and 1 case series) were included in the review after applying inclusion and exclusion criteria and performing qualitative assessment of the studies. 16 patients were included in our study after the completion of the qualitative assessment.

Inclusion Criteria:  English language, All ages, human, case report, case series.

Exclusion Criteria: Languages other than English, non-confirmed cases, incomplete studies.

 

RESULTS:

Selected articles included patients with endoscopically confirmed EIP and presented with complains of dysphagia.

1)Around 69% patients were above 50 years of age, and 31% patients were below 50 years of age.

2)There is a higher prevalence of EIP in males with 75% of patients being male with only 25% of patients being females.

3)Around 81.25% of the patients had preexisting co morbidities and risk factors like Diabetes Mellitus, Hypertension, Chronic GERD, alcohol dependance, opioid dependance, etc.

4)A total of 56.25% patients presented with a complication of stricture formation and were treated with endoscopic dilatation for symptom relief but presented with recurrence.

5)56.25% patients were chronic smokers which denotes it as an important risk factor.

6)3/16 patients had coexisting candida infection which was cured with antifungal treatment.

 

CONCLUSION:

Prevalence of EIP was evident in older age group with a male gender preponderance, history of smoking and preexisting comorbidities. Esophageal stricture is one of the common complications of EIP
